Missouri Form 4669: A Property Representative’s Guide

What Form 4669 is
Form 4669 is Missouri’s Abandoned Property Report. The Missouri Department of Revenue manual describes it as part of the process for qualifying private-property removals that are not authorized by law enforcement. The form is a legal declaration, and the current facts and requirements control.
Form 4669 is not a Kansas form. If the property is in Kansas, begin with the Kansas private-property process.
Who is involved
Property representative
The current Missouri materials identify the owner, lessee, or property/security manager in the property portion of the process. The current form and statute should be reviewed for the precise requirements that apply to the role and situation.
Towing company
The towing company has separate completion, delivery, notification, storage, and record responsibilities under the official process. Authorized Impound does not present the property section as a substitute for those duties.
Law enforcement
The official materials describe notification, inquiry, record, and signature functions for the jurisdictional agency. Linking to an agency does not imply that Authorized Impound is affiliated with or controls it.
Three official circumstances to review
Missouri’s current statute and August 2025 manual describe different circumstances, including signed property, certain owner-occupied residential property, and other private property after specified notification periods. They also include presence, signage, notification, timing, and role requirements that cannot be reduced to one universal rule.
Do not pre-sign, backdate, copy a signature, or treat a general towing agreement as automatic authority. Review the current official materials for the exact property and circumstances.
